Specialty Clinics
Specialty Clinics at SFMC are designed to support the patients of SFMC with chronic medical needs. Access to the specialty clinic is by referral only from providers at the acute care clinics. Patients participating in our specialty clinics have a “Patient Responsibility” agreement that must be agreed/signed upon prior to enrollment in the program.
Diabetic Clinic
- Fourth Tuesday each month
- 5:30 – 8 pm
- Group Meetings
- Individual consultations available with provider/RN
